* Only persons with confirmed appointments from Friday, March 19 will be administered a vaccine. Second doses only.

 

IMPORTANT!
What to Bring to Your Appointment

  • Bring a printed and completed copy of the Patient Intake you received by email.
  • Bring a photocopy of your ID card (driver’s license or passport) and insurance card.
  • Bring your vaccination record card to your second dose appointment.
  • A pen for writing; this will be required to fill out the forms we provide to you during your appointment.

Failure to bring these items may result in a significant delay to your appointment.
Persons without a vaccine record card will have a longer wait time.

Additional Information

  • Don’t arrive more than 15 minutes before your appointment time. This helps keep the vaccination process and wait time easy for everyone.
  • Plan and dress for the weather. After you get your vaccine, wait in the designated areas for 15 to 30 minutes before leaving. This helps us watch for any reaction.
  • Choose clothing that makes it easy to get to your arm for the vaccination.
  • Restrooms will be open for persons receiving a vaccine (only).

 

Where to Find a Second Dose Appointment If You Cannot Attend

If you are unable to keep the time scheduled, you may arrive at a different time but will be placed in second priority to all persons arriving on-time.

If you are no longer able to arrive to the North Metro Church campus on April 9th, you can search for a COVID-19 vaccine provider on the Georgia Department of Public Health website. Please click the link below to find a location.

Please do not call the pharmacy to ask questions about the vaccine or to request another appointment time. Per the terms of agreement, all persons who received a vaccine on March 19th MUST return on April 9th for their second dose. No other appointment days will be available for the Pfizer vaccine.
